At the start of a clinic session, the clinician opens a blank draft, confirms the correct patient chart in the record system, and captures the note in a separate workspace. The draft includes the history, relevant examination findings, test results already known to the clinician, and the plan. Before anything is pasted into the medical record, the clinician reviews names, numbers, medications, laterality, and follow-up language for accuracy. The final text is then copied into the appropriate chart and signed in the record system.

Method

  1. Speed uses the number of reference words divided by elapsed minutes.
  2. Accuracy uses a word-level edit-distance comparison after lowercasing and removing punctuation.
